01Hydration support

Sodium & electrolytes

Many people with POTS have low circulating blood volume. A clinician-directed fluid and sodium plan can support blood volume and reduce orthostatic symptoms. This does not necessarily mean your blood sodium is low. [1]

Before you supplementAsk your clinician for a daily fluid and sodium target. Compare sodium per serving, not just “total electrolytes.”

Iron & ferritin

Low iron stores have been reported in some people with POTS, including adolescent studies. Iron deficiency and anemia can also contribute to fatigue and rapid heartbeat. A CBC and ferritin can help guide next steps. [1, 2]

Before you supplementUse iron only when recommended after testing. Food-based iron is not automatically safer or better absorbed.

03Individual needs

Vitamin D

Vitamin D supports bone and muscle health. Low levels can occur with or without POTS; evidence does not establish vitamin D supplements as a POTS treatment. Discuss testing if your history or risk factors suggest a need. [3]

Before you supplementK2 is not required for vitamin D to work. Check with your prescriber before taking K2 if you use warfarin.

B12 & other B vitamins

B12 and folate deficiencies can cause anemia; B12 deficiency can also affect nerve function. Evaluation may be especially helpful with a restricted diet or absorption concerns. A B-complex is not a standard POTS treatment. [4]

Before you supplementTreat confirmed deficiencies individually. Avoid stacking high-dose B vitamins; long-term excess B6 can damage nerves.

Magnesium

Magnesium supports normal muscle and nerve function. POTS alone does not establish magnesium deficiency, and supplements are not a proven POTS treatment. Consider your diet, medical history, and tolerance first. [5]

Before you supplementSupplemental magnesium can cause diarrhea. Kidney disease and some medications require extra care. Dilemma Foundation also contains magnesium.

Digestive comfort

Nausea, bloating, constipation, and changes in stomach emptying can occur alongside POTS. Persistent symptoms deserve evaluation. They do not automatically mean poor nutrient absorption or an enzyme deficiency. [6]

Before you supplementDigestive enzymes are not a routine POTS treatment. Choose support based on the cause of your symptoms.

Bring this to your next appointment

Ask about: your fluid and sodium targets; whether a CBC and ferritin are appropriate; B12 or folate testing based on your history; and vitamin D testing when indicated.

Bring: your medication and supplement list, recent labs, and notes about your symptoms, meals, and digestion.

Keep it simple: select products with your clinician and avoid overlapping ingredients.

Good questions to ask

Before you add something new.

Do I need all of these supplements?

No. A POTS diagnosis does not mean you have every deficiency listed. Your clinician can help decide what to test and whether a supplement is useful.

How does Dilemma Foundation fit into this guide?

It combines electrolytes, magnesium, creatine, and D3 + K2. Its sodium content may cover only part of your prescribed daily target. Review the label before adding separate magnesium or vitamin D products. Creatine is included in the formula, but is not an established POTS treatment.

What if supplements bother my stomach?

Tell your clinician, especially with ongoing nausea, vomiting, weight loss, or trouble eating. The cause of the symptoms matters. An enzyme blend is not a substitute for evaluating stomach emptying or other digestive conditions.
